Case Management Services
Our participants are eligible for case management services. Case management services address some of challenges that our participant may have. Our CMS promotes and encourage community independence.
Case Management Services include:
- Assistance with identifying stable and/or suitable housing
- Assistance with physical health care management
- Exploring eligibility for entitlement benefits such as:
- Food stamps
- Comprehensive health insurance
- Soc Sec Income and disability benefits
- Assistance with identifying gainful employment and applying for job opportunities
- Assistance with managing daily life tasks to include:
- Appointment setting
- Personal grooming
- Time management
- Personal budgeting and finance
- Legal support and advocacy
We promote the following with every program participant:
- The promotion of both physical and mental health and wellness
- The integration of independent living and life skills
- The benefits of living a clean and sober lifestyle
- Becoming an active positive member of their family and community
